What is the proper procedure to notify a restaurant of problems when the manager is not responsive to concerns? On one occasion, the dish served was not what had been ordered. The waiter was notified, but did not respond. On checkout, the manager/ owner was notified and the response to the customer was that she should have notified the waiter. No apology given. At another establishment, on more than one occasion I have seen an order lost that is discovered after everyone else has been served. Each time there has been no concern or apology given for the inconvenience by the management. So what to do when the usual people to notify are no help? As a customer, I no longer take my business to those two places.

Sounds like you did the right thing, with maybe one exception. You might have asked to speak with the manager/owner when the waiter did not respond. I would think that they should have resolved it then.
There is much room for mistakes from the waiter/waitress misunderstanding the order, to the cook misreading the order, to just putting the wrong order on the wrong serving tray.

I too have faced similar situations, and kept quiet thinking the waiter would loose the job.
But lately I have notice if you complain, still they act as if we are at fault.
Sometimes even the superiors do not respond to it.
My only advice is to talk about it with you circle of friends , relatives who patronize them. Stop patronizing, may be over a time of period they would realize.

You should name the restaurants here. Give your review so others know to stay away. If the owners are smart they will respond where they have an audience.
